Please fill out this form to terminate a student employee whether voluntary or involuntary. If this is voluntary termination/resignation fill out the questions as asked. If this termination is for cause/involuntary please contact [emailprotected] to set up a meeting to go over proper procedure before terminating the student if you have not done so already. You will be able to set up a meeting request with Human Resources with this form. Notification of terminations need to be filled out ASAP once you are aware of this students leave from employment.
